Falls View Academy Preschool appreciates your interest in our school.
The first step in your child’s future is to fill out our simple application form. There is a $50.00 application fee, $40.00 per child if applying for more than one child in the same family.
You can fill in the form to the right or print out the PDF, fill it out and send it to us (mailing instructions are included with the pdf form)
We recommend that you get an early start and fill out your application as soon as possible.
- Please read and fill in all information on application form.
- A child must be three on or before December 1, 2012 to be admitted into a three year old class. A child must be four on or before December 1, 2012 to be admitted into a four year old class.
- Maximum class sizes are: 12 for three year olds and 12 for four year olds.
- Classes will begin on September 5, 2012 and end the week of June 18, 2013.
- Applications will only be processed by the registrar if they are received by mail. They will be processed by postmark. Applications will be accepted on a first come first served basis. Acceptance to the class of your choice is contingent upon the space available. Early applicants are most likely to receive the class of their first choice (please include your choice).
- Notification of acceptance and class placement will be made by mail for children not presently enrolled. Families of presently enrolled children will receive their confirmation as soon as the application is processed. Waiting lists are made when a class is full. Be sure to indicate on you application if you wish to be put on such a list.
- At the time of notification you will receive a health and immunization form if your child is entering our school for the first time. This form must be completed by your doctor and is required by law. Therefore, no child may attend FVA unless these records are on file with our school.
- Tuition payment is required in advance of each quarter. A $100 non-refundable deposit is required to reserve a space for you for the upcoming year and will be applied to your tuition. The payment schedule is $100.00 due June 1 and then the remaining tuition is due Sept. 1, Nov. 1, Jan. 1, and March 1.
